Mammogram Giveaway

For the second year in a row Bay News 9 is teaming up with clinics and hospitals around the Bay area for Breast Cancer Awareness month.

Each Monday in October we're giving away free mammograms and free three-month Curves memberships. Simply fill out the contest entry form below and click submit to enter the giveaway.

Winners will be randomly drawn each Monday at noon and winners will be announced during our Health Team 9 Medical Beat Monday nights at :53 past the hour. In addition, a Bay News 9 representative will contact the winners.

How to Enter:

Enter online at http://www.baynews9.com.
Deadline to enter is 12 p.m. on October 31, 2005.
Bay News 9 and/or our promotional partners are not responsible for late, lost or misdirected e-mails.
Only one entry per person.
No purchase necessary.
All entries become the property of Bay News 9 and will not be acknowledged or returned.
Entrants need not be a cable subscriber to win.
Applicants must be at least 18 years of age.
Applicants must not have had a mammogram in the past 12 months.

Winner Selection:

Winners will be selected at 12 p.m. on October 3, 10, 17, 24, 31, 2005.
Odds of winning depend on the number of eligible entries submitted.
All prizes will be awarded.
Need not be present to win.

Prize:

There is a total of five grand prize winners each week. Each winner will receive a mammogram and free three-month Curves membership.
One prize will be awarded to each winner.
The aforementioned prize is the only prize being offered. Not redeemable for any other cash or prizes.

Eligibility:

Employees of Bright House Networks, Bay News 9 and all sponsoring companies, their advertising and promotion agencies and the immediate family members of each are ineligible.
Void where prohibited by law.
Entry constitutes permission to use winner's name, e-mail, city of residence, quotes, utterances and likeness for promotional or business purposes without further compensation except where prohibited by law.
Bright House Networks, Bay News 9, affiliates and parent companies assume no responsibility or liability for damages, losses or injury resulting from acceptance or use of any prize.
Winners are responsible for all local, county, state and federal taxes applicable to the prizes.
