A preliminary report from the medical examiner’s office has been released on Chad Wolfe, who was found dead in an elevator shaft at Tampa International Airport in March.

The medical examiner says the 31-year-old died from a broken neck and a spinal cord injury. His death was ruled an accident.

He also had an "oil-like substance" on his hands, the report noted.

The report said "It appears [Wolfe] forced open [the] elevator door to gain entry into the elevator shaft,” quoting a detective.

Wolfe had flown into Tampa with his girlfriend to attend Daytona Bike Week. She reported him missing after he didn't return to the baggage claim area where they'd agreed to meet.

Before the incident, surveillance cameras caught him acting strange; climbing a potted plant, and drinking vodka while he walked to the terminal.

The report also shows Wolfe, a mechanic from Pennsylvania man, had an empty bottle of Xanax in his pocket when a maintenance worker found his body.

The full autopsy report, including toxicology results, will be released in a few weeks.
