The Polk County Sheriff's Office is investigating a crash that killed two people.

The crash occurred Wednesday night at about 8:20 p.m. in Lake Hamilton. 

The Sheriff's Office said an SUV and a Mazda car were driving side by side northbound on U.S. 27. 

For an unknown reason, the car hit the left side of the SUV, causing it to slide and then flip into a ditch, a Sheriff's Office report said. 

Two people in the back seat of the SUV were ejected from the vehicle and died. They were identified as Cobra Frost, 31, and Wyatt Singleton, 61. 

According to the report, Singleton would have turned 62 Thursday. 

Officials said the driver of the car stopped and cooperated with deputies. 

No one in the car was seriously hurt; however, a 15-year-old passenger was treated at a hospital for minor injuries. 

The investigation is ongoing.
